Piggyback IOL Implantation to correct Refractive Surprise Dr Suresh K Pandey.mpg In this video, Dr Suresh K Pandey and Dr Vidushi Sharma demonstrate management of refractive surprise (minus 5 diopter, operated elsewhere) by implantation a minus 6 diopter Sensar IOL in ciliary suclus of 48 years old male at Suvi Eye Institute and Research Center, Kota, India. First reported by Johnny Gayton, MD, in 1993, piggyback placement of an IOL can be a successful strategy for correcting residual refractive error. Placing an additional IOL in the eye is an effective way to correct a residual refractive error, particularly high degrees of refractive surprise. With the original IOL placed in the capsular bag, the piggyback IOL is traditionally placed in the ciliary sulcus to avoid interenticular opacity. These IOLs can be implanted during cataract surgery or clear lens extraction and IOL insertion (primary piggyback implantation) or as a secondary procedure following the initial IOL implantation (secondary piggyback implantation as in this case). IOL Power calculation for piggyback IOLs is based on residual refractive error and can achieve highly accurate outcomes. The traditional geometrical shape is that of a triangular prism with a triangular base and rectangular sides, and in colloquial use "prism" usually refers to this type. Some types of optical prism are not in fact in the shape of geometric prisms. Prisms are typically made out of glass, but can be made from any material that is transparent to the wavelengths for which they are designed. A prism can be used to break light up into its constituent spectral colors (the colors of the rainbow). Prisms can also be used to reflect light, or to split light into components with different polarizations. Light changes speed as it moves from one medium to another (for example, from air into the glass of the prism). This speed change causes the light to be refracted and to enter the new medium at a different angle (Huygens principle). This 45 years old patient presented to us with refractive error of +16 D Sph. +1.50 D Cyl. @ 170 degree (BCVA 6/12, due to amblyopia). IOL power calculation using Holladay 2 formula was 36 Diopter. The postoperative refraction was +0.25 D Sph (BCVA 6/12). We have planned to implant AcrySof Toric IOL (30 Diopter, T3 model, maximum power available) in the capsular bag and compansate 6 Diopter by implantation of a monofocal IOL (Flexifold) in the ciliary sulcus. While performing piggyback IOL implantation, the previous practice of stacking two acrylic lenses in the capsular bag has since been abandoned due to occasional problems with interlenticular opacification and reduced visual acuity. The residual IOL power originally calculated for placement in the capsular bag will need to be adjusted only if the IOL power of second IOL (intended for ciliary sulcus) is more than 8 Diopter. It is recommended that different IOL materials, in different locations, be used for the creation of a polypseudophakia lens pair.
