Laparoscopic Sleeve Gastrectomy performed by Dr. Sergey Lyass Laparoscopic Sleeve Gastrectomy performed by Dr. Sergey Lyass in a morbidly obese female with BMI 65. Is a new procedure that induces weight loss by restricting food intake. With this procedure, the surgeon removes approximately 85 percent of the stomach laparoscopically so that the stomach takes the shape of a tube or "sleeve." It is larger than the stomach pouch created during Roux-en-Y bypass—and is about the size of a banana. The stomach is restricted by dividing it vertically and removing more than 85% of it. This part of the procedure is not reversible. The stomach that remains is shaped like a banana and measures from 2-5 ounces (60-150cc) depending on the surgeon performing the procedure. The nerves to the stomach and the outlet valve (pylorus) remain intact with the idea of preserving the functions of the stomach while reducing the volume. By comparison, in a Roux-en-Y gastric bypass, the stomach is divided, not removed, and the pylorus is excluded. The Roux-en-Y gastric bypass stomach can be reconnected (reversed) if necessary. Note that there is no intestinal bypass with this procedure, only stomach reduction. Expected Weight Loss: Most patients can expect to lose 30 to 80% of their excess body weight over a 6 - 12 month period with the sleeve gastrectomy alone.

Adenomatous polyp of pyloric. Endoscopic diagnosis. In patients 56 years and during the endoscopic study using endoscopic system videoinformation OLYMPUS - EVIS EXERA II 180, was identified pyloric polyp. Polyp hung in the duodenal bulb. To perform a biopsy the polyp had to pull over the head in the lumen of the stomach. Histological conclusion: simple adenomatous polyp.
10.126GS Tailored Surgery for T2 Diabetes Mellitus: Technical Feasilbility in a Porcine Model TITLE: Tailored Surgery for T2 Diabetes Mellitus: Technical Feasilbility in a Porcine Model Objective : Bariatric surgery cause Type 2 Diabetes Mellitus (T2DM) resolution rates in the morbid obese patient of 95.1 and 80.3% for Biliopancreatic Diversion and RYGB. Initial experiences in the management of T2DM patients with BMI [less than] 35 are promising and different surgical procedures have shown improving rates over 90%. Our objective is to evaluate the technical feasibility of a procedure combining sleeve gastrectomy plus pylorus preservation duodeno -- jejunostomy (SG-PPDJ) in a swine model. Theoretical goals of this procedure are: 1. Decreased ghreline levels. 2. Duodenal diversion. 3. Distal intestine stimulation. 4. Weight and caloric offert control. Methods : After ethics committee approval, five healthy Yorkshire pigs ( 15 to 20 Kg) were selected. Through four ports of 12 and 5 mm the greater curvature of the stomach was isolated 3-cm distal to the pylorus with harmonic scalpel. The duodenum was then sectioned with stapler preserving the vasculature. A gastric tube was next created. An end to side hand-sewn duodenojejunostomy anastomosis finished the procedure. Diet was reassumed the next day after surgery Results : The surgical average time was 45.8 + - 14. 6 minutes. One pig died postoperatively due to malignant hyperthermia. This dog presented to East Stroudsburg Veterinary Hospital for vomiting and refusing to eat or drink anything 2-3 days after the dog had ingested some bristle-brush type pipe cleaners at home. This particular dog, a young male Bull terrier has already developed the dangerous bad habit of chewing things around the house, as well as swallowing many of the items he's chewed. This dog has already had multiple episodes of vomiting, diarrhea, loss of appetite and other problems which have previously resolved on their own without veterinary intervention. However, this time the dog appeared to be declining and the owner brought the dog in for a thorough examination and abdominal x-rays, which revealed material that appeared to be lodged in the dogs gastric pylorus. The gastric pylorus is the portion of the stomach where there is a type of natural valve where the stomach narrows down and empties into the small intestines. This narrowing of the gastrointestinal tract is frequently an area that objects get stuck after being swallowed by dogs or cats. Some of the most common items that get stuck in this area as reported by Veterinary Pet Insurance claims are socks, bones, corn cobs, underwear, pantyhose, rocks and balls, not necessarily in that order. Totally Laparoscopic Pancreaticoduodenectomy (Laparoskopik Whipple) Dr. Mehmet Kaplan, Gaziantep, Turkey Although the rapid development in laparoscopic surgery, pancreaticoduodenectomy is still one of the most discussed and technically demanding surgery performed only at few centers in the world. This report describes a totally laparoscopic pylorus-preserving pancreaticoduodenectomy for a tumor of ampulla of Vater with a successful outcome, representing the first description of this laparoscopic procedure in Turkey. The patient was a 58-year-old male patient with the diagnosis of ampullary tumor. The operation was performed with 5 ports. Following resection, all anastomoses were made intracorporeally. A double layer pancreaticogastrostomy, an end-to-side choledochojejunostomy, a two layer gastrojejunustomy and drain placements completed the operation. The operating time was 510 minutes, and estimated blood loss was 350 ml. Surgical margins were negative and number of retrieved lymph nodes was 16. The patient was discharged on postoperative day 6 without complication. This case demonstrates that laparoscopic pancreaticoduodenectomy is a feasible operative procedure in carefully selected patients. This technique can achieve adequate margins and follow oncological principles. However randomized comparative studies are needed to establish the superiority of minimal invasive surgery over traditional open surgery. Stomach Ulcer Video This is a video performed by Joseph S. Galati, Houston, Texas, on a young patient with chronic abdominal pain. After an evaluation with blood work and an ultrasound of the abdomen, an upper endoscopy was performed that revealed a large gastric ulcer in the antrum/pylorus of the stomach. Several smaller erosions were also noted in the stomach.
