  • Dr Ravish Patwardhan Surgical Video - Back Pain Relief - Artificial Disk Dr Ravish Patwardhan Helps Relieve a man's pain - TLIF ACDD Dr. Rhett McLaren wound up in an emergency room after experiencing severe pain in his back more than his legs, and Dr. Ravish Patwardhan was consulted for the first time. Already having had an operation elsewhere by another surgeon for his low back, he sought relief when other conservative measures had failed. He had already also had work-up done by another local doctor, which had determined the level of his problem as involving the lower lumbar interspace. He sought Dr. Ravish Patwardhan's assistance, as Dr. Ravish Patwardhan performed the operation he had been contemplating already as a patieint, but in a minimally invasive manner with smaller incisions; as a consequence, Dr. McLaren would be able to return to work sooner). The operation, a minimally invasive "transforaminal lumbar interbody fusion" or TLIF, was performed by Dr. Ravish Patwardhan for his severe back pain. Dr. Rhett McLaren had already had a lumbar diskogram performed by another physician, which localized the pain to the indicated level. To do this the minimally invasive way, Dr. Ravish Patwardhan utilized continuous X-ray guidance (called fluoroscopy) in two planes, so that the pedicles could be entered without extensive muscle dissection. By placing the screws over wires, Dr. Ravish Patwardhan avoided the pain which would come from cauterizing muscle; also, by using sequentially dilating tubes, stacked ...

Shows a lateral view of a normal and a degenerate L4-5 facet joint during flexion--extension. Also shows a superior view of the L4 nerve root during the natural progress of L5 facet joint arthropathy leading to foraminal stenosis. INTERVERTEBRAL FORAMEN The intervertebral foramen is formed posteriorly by the superior articular process of the vertebra below and the inferior articular process of the vertebra above, anteriorly by the vertebral bodies and the intervening intervertebral disc, and superiorly and inferiorly by the respective vertebral pedicles. The intervertebral foramen provides a protective exit tunnel for the spinal nerve to leave the spinal c***. FORAMINAL STENOSIS Foraminal stenosis refers to narrowing of the intervertebral foramina. It is commonly caused by a degenerative articular process enlargement posteriorly, anteriorly by posterolateral intervertebral disc bulging and posterolateral vertebral body lipping (osteophytes), and superiorly by the vertebral pedicle that moves inferiorly with intervertebral disc dehydration and collapse during degenerative disc disease. Less common causes of foraminal stenosis are spine tumours and infections, an expanding cervical synovial cyst and synovial chondromatosis. • TeamCirisano Teaching Series: Ovarian Tumor Removal, Laparotomy Very large ovarian tumors may require an abdominal incision to accommodate complete removal. The larger incision may be needed to allow separation of the abnormal tumor tissue from the normal organs within the abdomen and pelvis. Technological advances in electrocautery allow these procedures to be completed with minimal blood loss. The bovie electrocautery pen is used to divide adhesions between the tumor mass and normal tissues such as loops of bowel and colon. The Ligasure bipolar cautery instrument is utilized to divide vascular pedicles such as the blood supply of the ovarian tumor, anatomically called the infundibulo-pelvic ligament. The gallbladder trocar can also be used for controlled drainage of extremely large cystic masses, utilizing the purse string suture technique to seal the aspiration site following drainage.

The spine as a whole is a part of the skeleton that provides support on the one hand and flexibility on the other hand. To form the spine as a functional entity the single vertebral bodies must be interconnected both firmly and flexibly. The spinal disks play an important role as buffers between the vertebral bodies. They dampen shocks and blows. Without these shock absorbers the vertebral bones would grind. If the disk is overworked and under constantly too high pressure, it will get brittle, loose its elasticity and decrease in height. With increasing degeneration the vertebral joints will get instable and can slip. Nerves can be compressed and damaged. This leads to severe pain.

  • The SPECIAL™ Technique for Nerve Preservation During Robotic Prostatectomy Prostatic pedicle control during robotic prostatectomy has been performed in a variety of ways. Prior series have used clips or bipolar electrocautery. The technique of bulldog clamping of the pedicle prior to transection, followed by over-sewing, has been reported. We present the novel SPECIAL technique. The SPECIAL technique provides excellent exposure and vascular control while avoiding trauma to the neurovascular bundles. In addition, this method obviates the need for bulldog clamping with subsequent tedious over-sewing of the prostatic pedicle. These are probably the factors leading to the validated early return of erectile function.

  • Herniated Disc Surgery on Olympic Gymnast Scott is one of the top gymnasts in the world. He suffered a herniated disc in a training accident. He went to Dr. Delamarter who rebuilt his back with the pedicle screw, which is sometimes used as an adjunct to spinal fusion surgery, it provides a means of gripping a spinal segment. The screws themselves do not fixate the spinal segment, but act as firm anchor points that can then be connected with a rod. The screws are placed at two or three consecutive spine segments (eg lumbar segment 4 and 5) and then a short rod is used to connect the screws. This construct prevents motion at the segments that are being fused. The treatment allowed Scott to compete for the next Olympics within one year. Visit or call 888-774-6376 for more information.

  • Lumbar Spine Pathology Vertebral Body Anterior Wedge Compression Fracture medical education art www.medilaw.tv Lumbar Spine Pathology Vertebral Body Anterior Wedge Compression Fracture medical education art. Shows the causes and effects of a compression fracture. The vertebrae are very strong, but they can be fractured just like any other bone. A compression fracture can occur when normal force is applied to a weakened vertebra, as in osteoporosis, infection or a cancer deposit, or if excessive force is applied to a normal vertebra, as in a high fall or jump or a motor vehicle accident. BURST FRACTURE If the compression force occurs vertically, it can cause a burst fracture. As the fractured vertebral body decreases in height, it expands in depth and width, and bone fragments are likely to irritate or damage adjacent structures such as the spinal cord and spinal nerve roots. This is a potentially debilitating injury due to spinal cord damage and paralysis. The spine can be divided into three columns -- the anterior column (anterior half of vertebral bodies), middle column (posterior half of vertebral bodies), posterior column (vertebral arch -- pedicles, lamina, facet joints). If two or more columns are damaged, the area is unstable and a surgical fixation procedure will be required. Lumbar Spine Pathology Vertebral Body Anterior Wedge Compression Fracture medical education art. WEDGE FRACTURE If the excess compression includes forward flexion of the spine, this concentrates the compressive force on the front of the vertebral body, which collapses and forms a wedge ...

The animal was a 12 kg *** owning pyometra. The hemostasis of the ovarian pedicles was carried out using a 5 mm Lina Tripol Powerblade, which provide excelent both coagulation and cut function. The uterine body and vessels were ligated using poliglecaprone 25 no. 0 and Miller's knot. Omentopexy of the uterine stump was performed. The patient convalesced well and developed no post operative complications. Potential advantages of such surgical technique includes short operating time and better recovery for animals presenting moderate or severe uterine infection.
  • TeamCirisano Teaching Series: robotic hysterectomy for uterine fibroid The role for robotic surgery includes the laparoscopic hysterectomy procedure for uterine myomata also known as fibroid tumors. Depicted here is the procedure which may be indexed to consist of 3 primary steps that secure blood supply within vascular pedicles. Additionally, 3 ancillary or secondary steps must be completed which allow development of spaces that usually are less vascular ( avascular ). Once all six steps are completed, the specimen may be delivered via the vaginal incision also termed a colpotomy incision. The primary steps consist of division of the ovarian artery ( right and left ), the uterine artery ( right and left ), and the blood supply to the vaginal apex ( anterior and posterior ). The secondary steps consist of division of the round ligaments ( right and left ), the uterosacral ligaments ( right and left ), and development of the bladder flap also called the vesicovaginal space. Following delivery of the specimens, the vaginal incision is closed with two stitches using robotic assisted wristed instruments.

  • Lumbar Spine Pedicle Screw Fixation Fusion lawyer 3D animations www.medilaw.tv Lumbar Spine Pedicle Screw Fixation Fusion lawyer 3Danimations. Illustrates the surgical technique for performing a laminectomy and pedicle screw fusion. This procedure is used to decompress the spinal cord and immobilize an intervertebral disc or facet joints that are causing uncontrollable pain. Pedicle screw instrumentation is used to ensure stability while fusion occurs. There are many different techniques to achieve the same end result, a pain-free, stable, anatomically positioned bony fusion. However, the basic procedure illustrated here is common to all pedicle screw fusions. Also shown is the patient position, skin preparation and incision, the surgical approach, the removal of the lamina, the insertion of the pedicle screws, rods and bone graft, x-ray position checks and finally wound closure. A spinal fusion is done to join two vertebrae together to make one large bone. The surgeon roughens up the external surfaces of the two vertebrae to make the body's natural repair system think that one large bone has broken. The surgeon then adds bone to fill the gap. The body then joins the mass together, like a normal fracture. While the bone is healing, it is held still by screws and plates or rods. Full fusion takes three months. Bone chips can be taken from your hip at the time of the operation, and then grafted onto your vertebra. Alternatively, bone can be harvested from other patients and stored until needed in a bone bank. Using bone from the bone ...
