Some times even H. Pylori infection can develop in these ectopic gastric mucosa islands. It can be found in about 5% of infants and up to 10% of children. Its presence is estimated at around 8% in adults. Traditionally, Barrett oesophagus is considered a distinct entity from oesophageal inlet patch. Barrett oesophagus is an acquired precancerous lesion and the cell origin probably involves multipotential undifferentiated cells. Inlet patch is considered to be congenital and has no malignant potential. However, recent studies have challenged these concepts. More and more endoscopic series have demonstrated a frequent association of cervical inlet patch with Barrett oesophagus. There is a debate regarding to treat or not to treat these patches using EMR or HALO radioablation. If the lesion is associated with Barrett's oesophagus in a symptomatic patient the present tendency is to treat.

Dr Persson and colleagues used data from the HIV/AIDS Cancer Match Study, a linkage of 15 US population-based HIV/AIDS and cancer registries with data from 1980 through 2007. Overall, people with AIDS had a 6.9-fold increased incidence of stomach malignancies and a 2.7 times increased risk of oesophageal malignancies compared to the general population.

  • Thanks, Hitch. On the 30th of June 2010, Christopher Hitchens posted a short notice on Vanity Fair's website. He'd been diagnosed with oesophageal cancer. This is a very grave prognosis, since oesophageal cancer is usually discovered very late in the cancer's development. Hitchens, the famous author, atheist, commentator and polemic, had been planning to spend the next several weeks touring and promoting his recently published memoir, Hitch-22. In light of his diagnosis, he has cancelled his book tour to receive chemotherapy treatment. In July, Vanity Fair published the first piece by Hitchens since his diagnosis. In it, he mentions that the cancer has metastasised to his lung and lymph nodes. I regard Hitchens as one of my heroes. For all his bluster, he is undoubtedly one of the most important public intellectuals of our generation. Most know him only for his outspoken atheism, and many forget that he is also a great journalist and writer, and a staunch human rights activist besides. He is a man of uncommon integrity and strength of character, and the world would indeed be poorer without him. Here's hoping that he will be with us for several years, yet. This is my submission for The Thinking Atheist's support video. My thoughts are with Hitchens and his family during this difficult struggle.   • Fishbone Impaction at Lingual Tonsil Part I Fishbone impaction in digestive tract is relatively common and causes significant pain and difficulty in swallowing (dysphagia/odynophagia). Clinical examination may reveal the implicated foreign body in the oro- or hypo-pharynx, unless it has passed and impacted beyond the oesophageal opening. This video shows a fishbone which had pricked and anchored at the left lingual tonsil area. It was initially hidden by the epiglottis when examined by rigid endoscopy. However, fiberoptic laryngoscopy had made it visible and thereafter was successfully removed by using mini forceps passed through the side channel of the fiberoptic scope. The removal was performed as an office procedure after the patient throat anaesthetized (made numb) with lignocaine 10 percent spray. No adverse event occured during its removal.

  • Hiatal Hernia and Barrett´s Oesophagus Hiatal hernia is the most common cause of oesophageal reflux disease. A chronic reflux disease can cause an abnormal change in the oesophageal mucosa, called Barrett´s oesophagus which can be found in 5 - 15% of patients seeking help for reflux symptoms. Barrett´s oesophagus is a risk factor for oesophageal cancer. The risk is not high however - about 0.5% per patient year.
