It's normal for the red blood cells to break down, although the bilirubin formed doesn't normally cause jaundice because the liver will metabolize it and then get rid of it in the gut. However, the newborn baby will often become jaundiced during the first few days due to the liver enzyme that metabolizes the bilirubin becoming relatively immature. Therefore, newborn babies will have more red blood cells than adults, and thus more will break down at any given time. Kingdom Monera is made up of unicellular prokaryotes that lack membrane bound organelles and a membrane bound nucleus. Bacteria, the first organisms on earth, make up this kingdom. The bacteria can be heterotrophic, where it cannot make its own nutrients and instead obtains its nutrients from other organisms. Or, it can be autotrophic where it can produce its nutrients through phototrophs, which use light, or chemoautotrophs, which metabolize things like sulfur, salt, and iron. Kingdom Protista are eukaryotic with membrane bound organelles, a nucleus, and are mostly unicellular. Although some are multicellular for example, seaweed. Some Protista are good and others can be bad like some pathogens. Kingdom Fungi are sessile, unicellular and multicellular heterotrophs. Many fungi obtain nutrient molecules by extracellular digestion. They secrete enzymes that hydrolize cellulose into smaller components such as glucose. The small molecules follow a concentration gradient into the fungal cells then are metabolized. Many fungi live in symbiotic relationships which can be both beneficial, parasitic, or harmful.   • Reduced Effect from Plavix in Some Patients Through a new boxed warning, the FDA is alerting healthcare professionals about a subgroup of patients who cannot effectively metabolize the anti-platelet drug Plavix (clopidogrel). These patients, called "poor metabolizers," have little or no activity of the liver enzyme CYP2C19, which converts Plavix to its active form, so they may not experience the full anti-clotting benefits of the drug. Practitioners should know that tests are available to identify genetic differences in CYP2C19 function and thus identify poor metabolizers. They should consider using other anti-platelet medication or an alternative dosing strategy for these patients. And although raising the dose of Plavix in poor metabolizers can increase anti-platelet response, an appropriate dose regimen has not been established in a clinical trial.   • Reducing Chemotherapy Toxicity for Colon Cancer Patients Everyone responds differently to medicines. The dose of a drug that cures one person can be ineffective-or even toxic-in someone else. Although many non-genetic factors play a role in how an individual patient responds to a particular drug treatment, many differences in drug response come from genetic variation. Individualizing a drug's dose based on pharmacogenomic information can help a patient get the most benefit from a drug while minimizing side effects. Take the example of treating colorectal cancer with a chemotherapy regimen that includes Camptosar (irinotecan). A gene, called UGT1A1, produces an enzyme that metabolizes Camptosar. Variations in this gene can influence a patient's ability to break down the drug. About 10 percent of the North American population has a variation of the UGT1A1 gene that reduces their ability to metabolize Camptosar, leading to high blood levels of the drug and a higher risk of toxic side effects. If these patients are given standard doses of Camptosar, about half will develop severe neutropenia, which can be fatal. A test, called the Invader UGT1A1 Molecular Assay, can identify whether a patient has the genetic variant affecting the metabolism of Camptosar and thus would be at higher risk for developing severe neutropenia. The drug label says to consider lowering the starting dose of the Camptosar for those patients found to be high risk.
