Tumescent solution and lidocaine Traditional opinion is that maximum safe dosage of lidocaine is 7 mg/kg. In tumescent infiltration for our vein procedures, we are able to treat both of the lesser saphenous veins under that dosage. We use 500mg lidocaine and 0.5mg of epinephrine in 1L of sterile 0.9% NS, and have found that patients experience local anesthesia upto 24 hrs later. What that tells me is that systemic absorption is so low due to dilution effect and epinephrine, and absorption distributed over 24 hours that dangerous blood levels does not occur. I'd be interested to do a study on serum lidocaine level after evlt tumescence. Dr. Jeffrey Klein, a dermatologic surgeon from California has shown in his study that in tumescent liposuction using 1% lidocaine with epinephrine, up to 35-45 mg/kg has been found to be safe. I would like to see the data on evlt tumescent usage. Drawing lidocaine i was at work when i made this video, so there is no commentary...read the description for a general step by step of what you're seeing. my mom asked me if i was doing this safely...so i took my ipod to work and took a video of myself drawing two 1.5ml syringes of lidocaine. i work in a dermatology clinic and use this stuff every single day. lidocaine is a local anesthetic used to numb patients before procedures...in my case they are biopsies, both punch and shave. At first i am fumbling around getting out equipment that wasn't on the table when i started and what not, but the procedure is pretty straight forward. the red needle tip is called a blunt fill, it's significantly less sharp than a regular needle that is used only to draw up medications. The yellowish-looking tip that i screwed on after is the actual needle, an 18g (very small) needle about a half inch long used to actually inject the medication into the patient. the yellow plastic part covers the needle at all times when it is not being directed into the patient's skin. When i started the procedure you'll notice that i uncapped the lidocaine and then wiped it down with an alcohol swab...you don't have to do so with a brand new container but i make it a habit to do it always, and thus not forget to do it when you're re-using a vial of the drug. When drawing, it's best to inject the vial of medication with air, equal to the amount of fluid you would like to withdraw first, which is what you didn't see me doing ...
