  • Hepatitis B and Liver Transplant Study-Mayo Clinic Treatment to reduce recurrence of hepatitis B appears to improve liver transplant outcomes for some patients, according to a Mayo Clinic study. We found that in patients with hepatitis B, the number of patients who were listed for a second transplant within three years decreased by 50 percent between 1996 and 2005, says Ray Kim, MD, a member of the research team. Hepatitis B infection is a major cause of liver damage that can eventually lead to end-stage liver disease and the need for a liver transplant. In the past, hepatitis B recurred in some transplant patients, causing liver damage and the need for a second transplant. In the past 10 years, new medications have improved our ability to control hepatitis B, says Dr. Kim. Today, transplant recipients routinely are treated with antiviral therapy and hepatitis B immune globulin to reduce the risk of recurrence. We wanted to know if the medications protocols were making a difference in patient outcomes, he says. Researchers reviewed data of 31242 liver transplants in the United States which were provided by the Organ Procurement and Transplantation Network. The underlying reasons for transplantation were categorized as hepatitis B, hepatitis C or other. From 1996 to 1998, 6.5 percent of liver transplant patients with hepatitis B were listed for a second transplant. For 2003 to 2005, the number of liver transplant patients with hepatitis B listed for a second transplant was 3.3 percent — a roughly 50 percent reduction.

Hes celebrated birthdays, Christmases and New Years Days surrounded by nurses and doctors. Born with Primary Immune Deficiency (PID) NEMO, an extremely rare genetic mutation that caused his immune system to fail, Andys prognosis was dire. His parents were desperate to save him. With only 62 cases of PID NEMO worldwide, there was little medical precedent to help them. Andy received gamma globulin infusions every two weeks to boost his immune response and a daily ***tail of 13 different medications that kept him relatively safe. He had a very isolated childhood, as contact with the germs of the outside world could prove deadly. Childrens Hospital Boston immunologists recommended a stem cell bone marrow transplant to replace parts of Andys immune system. In October 2004, Andys baby sister Sofia became his donor. Stem cells from her umbilical cord and her bone marrow were used in the transplant.   • Hemolysis Risk when treating ITP patients with WinRho SDF Baxter Healthcare and Cangene Corporation are notifying healthcare professionals that intravascular hemolysis (IVH), which can be fatal, has occurred in patients with immune thrombocytopenic purpura (ITP) treated with WinRho SDF, an intravenous immune globulin product. Intravascular hemolysis can lead to severe anemia, renal failure, acute respiratory distress syndrome, disseminated intravascular coagulation, and multi-system organ failure. Elderly patients with co-morbid conditions appear to be at higher risk for developing acute hemolytic reactions. Here are the recommendations that now appear in the boxed warning for this product: • Closely monitor patients in a healthcare setting for at least eight hours after administering WinRho SDF. But be aware that IVH can occur even after that time. • Perform a baseline dipstick urinalysis. Repeat this at two and four hours after administration, and at the end of the monitoring period. • Instruct patients to watch for symptoms of IVH, including back pain, shaking chills, fever, discolored urine and hematuria. • If you suspect IVH, confirm this with laboratory tests, including plasma hemoglobin, haptoglobin, LDH, direct and indirect plasma bilirubin, and dipstick urinalysis. Finally, as a reminder, it is important to monitor all patients receiving intravenous immune globulin products for signs and symptoms of hemolysis, and to perform confimatory lab tests if they occur.   • FDA Approves First Immune Globulin for Subcutaneous Use FDA recently approved a new immune globulin product that is injected subcutaneously. The product, called Vivaglobin, is made by the German company ZLB Behring. Until now, immune globulin products have been administered either intravenously or intramuscularly. The approval of Vivaglobin provides a new way to administer antibody replacement therapy to patients with primary immune deficiency disease. This may be especially beneficial in patients who don't easily tolerate IV administration of IG because of poor venous access or serious side effects. Since Vivaglobin is injected on a weekly basis using an infusion pump, patients can self-administer the product at home. In clinical studies, Vivaglobin was well tolerated. The most common side effects were mild or moderate injection site reactions such as swelling, redness and itching. The contraindications for Vivaglobin are similar to other immune globulin products.

  • Avoiding Glucose Monitoring Errors Avoiding Glucose Monitoring Errors in Patients Receiving Other Sugars (February 2006) This story originally aired in February 2006. In this Special Edition of PSN, we are repeating some of the most important safety issues that continue to pose a public health problem. FDA issued a reminder about the potential for falsely elevated glucose readings in patients who are also receiving products that contain other sugars. These products include oral xylose, parenterals that contain maltose or galactose, and peritoneal dialysis solutions that contain icodextrin. Several patients have died because of this problem. Their glucose readings were very high, so they were given aggressive insulin treatment. The glucose readings were falsely high, however, and too much insulin was administered. The patients suffered hypoglycemic shock or irreversible brain damage and died. In these cases, the glucose meter was reading the patient's actual blood glucose level, plus the maltose that was contained in the IV immune globulin he was receiving. The readings were falsely elevated because the glucose monitoring device couldn't distinguish between glucose and other sugars. All glucose meters don't suffer from this problem. There are several kinds of enzymatic glucose monitoring methods that are used, and only one of those has this problem.
