Clinically, patients develop vesicles, pustules, and bullae which rupture to form erosions with excessive granulation tissue and crusting. These may evolve into exudative, vegetating masses with pustules and fissures. Lesions tend to be located in the intertriginous areas, in the scalp, and on the face. Oral mucous membranes are almost always involved. On histopathologic exam, one finds hyperkeratosis, papillomatosis, and acanthosis in addition to suprabasilar clefts with acantholytic cells and eosinophils. There is also typically a dense inflammatory infiltrate in the dermis composed of eosinphils, lymphocytes, and occasionally neutrophils. Direct immunofluorescence of perilesional skin demonstrates intercellular IgG and occasionally C3. Indirect immunofluorescence is positive for circulating IgG in the majority of patients. The course is similar to that of pemphigus vulgaris. However, spontaneous remission is possible.

To visualize this, imagine the roots of a tree growing and spreading until they actually uproot a sidewalk. Then imagine rainwater seeping up throughout the cracks. These abnormal blood vessels (the "roots) tend to be very fragile. They often grow, leak or bleed, causing scarring of the macula. This fluid is called exudate and wet AMD is sometimes called exudative macular degeneration. This damage to the macula results in rapid central vision loss. Once this vision is destroyed, it cannot be restored. There are several treatment options for wet AMD which can be very effective if applied early.

She was refered and very surprisingly in angiography no edema was seen. Question: why a clinical edema, but FA no edema? The aroused doctor wasn't happy with the retina and asked for OCT. An ERFP and a nearly full thickness hole were detected due to myopic staphylomatous enlargement of posterior sclera and traction, a retinoschisis in addition. Well edema, but no exudative one. Answer: traction & myopic retinoschisis! Vitrectomy and peeling resulted in a visual gain from 0.4 to 0.8; no latanaprost effect!
