Avandia and Diabetic Macular Edema (March 2006) Glaxo SmithKline has notified health professionals about reports of diabetic macular edema in patients receiving Avandia (rosiglitazone maleate) which is used to treat type 2 diabetes. These reports involved both new cases of macular edema and worsening of existing cases. In the majority of instances, patients also reported concurrent peripheral edema. In some cases, the macular edema resolved or improved when the drug was discontinued, and in one case it resolved after the dose was reduced. Repression Of Edema And Repression Of Blood Vessel Permeability with Systemic Enzymes It is known that proteases and peptidases are only absorbed in the intestinal area. These enzymes are mobilized directly to the blood and are not easily detectible in urine. Other enzymes with structural similarities have been reported to be absorbed through the intestinal tract. Chymotrypsin is transported into the blood from the intestinal lumen. Horseradish peroxidase can cross the mucosal barrier of the intestine in a biologically and immunologically active form. Several studies have appeared so far which refer to the systemic effects of orally given proteases and peptidases (eg serrapeptase), such as repression of edema and repression of blood vessel permeability induced by histamine or bradykinin. These enzymes also affect the kallikrein-kinin system and the complement system, thus modifying the inflammatory response. This is substantiated by reports of significant reductions in C3 and C4 complement, increases in opsonizing protein and reductions in concentrations of haptoglobulin, which is a scavenger protein that inhibits lysosomal protease. http

Diagnosis of ankle edema Ankle edema detection by ultrasound Skin edema will 'pit' only when there is considerable fluid collection. In early fluid retention, pitting would not be demonstrable by physical examination. This lady felt that her ankles were uncomfortable. It was difficult to exclude a joint or tendon disorder. Her BP and heart sounds were normal. There was no pitting of the skin. Ultrasound of the lower legs showed normal tendons and joints. However there was edema in the subcutaneous tissue in both legs. USS is much more sensitive than physical examination to confirm fluid collection in the skin Her edema was related to Norvasc prescribed for hypertension.

Macular Edema and Macular Ischemia Eyes with macular edema caused by retinal branch vein occlusion underwent masked evaluation of fluorescein angiography to determine complete macular perfusion vs incomplete macular perfusion (capillary dropout, ischemia). Cases evaluated as incomplete macular perfusion showed a greater frequency of improvement (91%) in visual acuity than did perfused cases (29%) (P = .003) after a mean follow-up of 39 months. Ischemic edema is often transient, and is associated with a good outcome in visual acuity (median final visual acuity was 20/30). Perfused macular edema has a poorer prognosis for visual acuity (median final visual acuity was 20/80). Macular ischemia is usually associated with a broken foveal capillary ring. Previous animal research on ischemic brain edema has shown that following brain ischemia, an intracellular and an extracellular hypertonic environment lead to intracellular and extracellular edema (cytotoxic edema), which is often followed by vascular protein leakage (vasogenic edema). A similar occurrence in ischemic retina could explain the transient edema reported herein, with good outcome in visual acuity following the spontaneous resolution of edema.
