The Origin of Breast Cancer This animation visualises research published in Nature Medicine (Vol 15, Issue 8, 2009) by the laboratory of Jane Visvader and Geoffrey Lindeman. The mammary gland is comprised of three main cell types; alveolar, ductal and myoepithelial cells. Stem cells can develop into any of the three cell types through a series of intermediate cell stages. One intermediate is the luminal progenitor cell, which develops into either alveolar or ductal cells. The paper describes how an aberrant form of a luminal progenitor cell is involved in the development of some forms of breast cancer. Insidermedicine In Depth - December 7, 2010 - Breast Cancer/Ductal Carcinoma In Situ Both radiation therapy and the anticancer drug tamoxifen reduce the risk of having a breast cancer event among women who have had surgery to remove ductal carcinoma in situ, according to an update of a long-term trial published in the Lancet. Here is some information about ductal carcinoma in situ: • It is an early form of breast cancer in which the disease has not yet invaded the breast • "Ductal" means that the cancer remains inside the milk ducts and "in-situ" means it has not spread anywhere else • While the condition can be successfully cured with surgery, women with this condition are at an increased risk for developing invasive breast cancer later on. Researchers from the University of London randomly assigned over 1700 women who had undergone surgery to completely remove ductal carcinoma in situ to receive radiation therapy, tamoxifen, both treatments, or neither. After an average follow-up period of nearly 13 years, radiation therapy was found to reduce the risk of developing any breast cancer event by 59%. The treatment also reduced the risk of developing invasive breast cancer in the originally affected breast by 68% and recurrent ductal carcinoma in situ in the affected breast by 62%. Tamoxifen treatment reduced the risk of developing another breast cancer event by 29%, the risk of developing recurrent ductal carcinoma in situ in the same breast by 30%, and a tumor in the unaffected breast by 56%. Invasive Ductal Carcinoma (Stage III) Marilynn Grossball heard about the Issels Medical Center after she received her diagnosis of Invasive Breast Cancer. She already had her mammogram, ultrasound, and core biopsy done and those returned with a diagnosis of Invasive Ductal Carcinoma. She came to Dr. issels before she did anything and he recommended that she get the lump taken out as the first step. She went ahead with the procedure and returned to with her results. Her biopsy results showed that her cancer had spread to both sentinel nodes. Her oncologist was strong and clear given the statistics and information, that Marilynn's best shot was to do big doses of chemotherapy, as well as Tamoxifen (because of her estrogen-sensitive cancer). The surgeon wanted to have all her lymphs taken out and advised that she see a radiologist as well. The radiologist gave her statistics about having 40% greater chance of living in 5 years if she does massive doses of radiation for her fast-spreading Type III cancer. Marilyn wanted to build her immune system, and detoxify and balance her body's systems, which made sense to her as a nurse and as a health educator. When she came to the Issels Medical Center in 2007, her cancer marker had doubled from the time of her lumpectomy, so there was reason to believe there was cancer in other places in her body. Here is some information about lobular type breast cancer: • It is the second most common type of breast cancer, after ductal breast cancer, accounting for about 10 to 15% of all breast cancers. • This cancer originates in the lobules of the breast, where milk is made • It can be either invasive, meaning it has spread beyond the lobules, or non-invasive, meaning it has not Researchers from Fred Hutchinson Cancer Research Center studied nearly 3000 postmenopausal aged 50 to 79 with breast cancer who participated in the Women's Health Initiative study. These women provided detailed information about their health and lifestyle at the outset of the study and were followed for up to five years. The investigators looked specifically at the relationship between the type of breast cancer the women had and the women's alcohol consumption, demographic and lifestyle characteristics, family history of diseases, and reproductive history. Women who drank at least one alcoholic beverage per day were about twice as likely to develop lobular breast cancer but had no increased risk for ductal breast cancer, which is a much more common form of the disease. Teri - Breast Cancer Survivor Breast cancer survivor Teri Johnson has taken the phrase "everything happens for a reason" to heart. And as practice manager for Nashville-based Tennessee Breast Specialists, Teri feels fortunate that she is able to share her story giving purpose to her own breast cancer experience by helping others through theirs. "My mother was just 51 when she died of breast cancer," says Teri. "Her experience was what gave me the fight to insist on getting the tests I needed." Teri had suffered from fibrous lumps in her breasts since she was 15 and was an advocate of self-breast exams. But because she had lost her mom at such a young age and because she had two young children of her own, Teri educated herself on the disease particularly on prevention and early detection. She insisted on getting a mammogram, even though she was just 29 at the time. The mammogram was able to detect ductal carcinoma in situ (DCIS), a form of breast cancer that forms in the milk ducts and can only be detected with a mammogram. She credits her breast surgeon, Dr. Laura Lawson, with providing a thorough and honest review of her treatment options. "We discussed chemotherapy, radiation and mastectomy," said Teri, adding that the risk of a recurrence with a bilateral mastectomy was only about 5 percent as opposed to 15 to 20 percent with chemo or radiation. Breast Ductal System Excision.Professor Melhem Novel Modification Excision of the major ductal system of the breast as described by Hadfield in 1960 is associated with several complications, such as loss of sensation in many women, nipple retraction in 10% and necrosis of aerola-nipple complex. In order to reduce such complications, we have modified the classical Hadfield technique. The basic difference is advancement of the aerola-nipple complex as a flap into a new bed immediately above the incision across the upper half of aerola-skin junction. The results of 46 operations performed for 33 patients (13 patients had bilateral operation) were satisfactory. After a mean follow up of 3.5 years, loss of sensation occurred in two patients (4%) epithelial necrosis of the upper half of the aerola in one patient (2%) and recurrence of discharge in another patient (2%). No episodes of wound infection or nipple retraction were reported in this study. The results demonstrate the effectiveness of this technique as an appropriate alternative to classical Hadfield operation
