  • New Treatment Approach to Rare Cancer Results in Prolonged Survival-Mayo Clinic Aggressive treatment of anaplastic thyroid carcinoma has dramatically increased survival in the small group of patients who chose to undergo it, say physicians at Mayo Clinic. Their findings will be presented at the annual meeting of the American Society of Clinical Oncology, to be held June 4-8 in Chicago. Mayo oncologists say this new approach reflects the need to aggressively treat metastasis even when this rare cancer seems to be confined to the neck. Historically, anaplastic thyroid cancer has been treated with surgery and radiation, but due to rapid spread of microscopic cancer, only between 10 and 20 percent of patients survive past a year. Because the prognosis for this cancer is so poor, Mayo physicians felt that a more aggressive pilot approach that added in chemotherapy early on was merited. Of 24 patients with newly diagnosed anaplastic thyroid cancer seen at Mayo Clinic between 2003 and 2007, 10 patients with local disease elected to pursue the aggressive approach. After surgery, the patients were treated with intensity modulated radiation therapy (IMRT), a more tailored form of radiation therapy than has been used before, and with aggressive chemotherapy (docetaxel and doxorubicin) in efforts to simultaneously control disease in the neck and forestall metastasis.   • ASH 2010: Key research presented at ASH 2010 Prof George Canellos, Harvard Medical School, Boston, USA, talks about the key research that has emerged from the 2010 ASH meeting in Florida. One trial of note compared the Stanford V treatment regimen with ABVD for patients with advanced Hodgkin's lymphoma and revealed that Stanford V was not superior to ABVD. Prof Canellos explains why this lack of superiority and the adverse effects associated with Stanford V's obligate radiotherapy indicate that ABVD is a more attractive treatment option. A second trial which avoids the use of radiotherapy involved patients with primary mediastinal large B-cell lymphoma. This trial demonstrated that treating patients with an intensified chemotherapy regimen of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine and prednisolone) and ICE (ifosfamide, carboplatin and etoposide), whilst only irradiating patients when it was necessary was associated with an extremely high survival rate. Prof Canellos concludes by discussing two trials demonstrating the effectiveness of targeting CD30 protein cells with the antibody SGN-35 (brentuximab vedotin) and one looking at treating post-transplant relapsed or refractory Hodgkin lymphoma patients with the histone deacetylase inhibitor panobinostat (LBH-589).

  • Ashley Hyde Appeal Ashley is a very happy and cheeky little boy. At the age of just 5 years he was diagnosed with stage 4 mycn amplified neuroblastoma on the 9th June 2008. Neuroblastoma is an aggressive childhood cancer. The disease is caused by abnormal neuroblasts or nerve cells which are special cells present in babies and children that help create the adult nervous system. Tumours originate from the sympathetic nervous system, which runs from the neck to the tailbone and can spread to other organs, bones and the bone marrow. If the cancer is diagnosed early, surgery and chemotherapy can be curative, but in 70% of cases the disease is not diagnosed until it has already widely spread, unfortunately this was the situation with Ashleys case. Upon diagnosis Ashley had a 12- 10 - 8 cm tumour above his left kidney and wide spread disease to the bone marrow in both legs, hips, shoulders and 1 arm. Ashleys little body started an incredibly hard battle to rid the neuroblastoma in it, Ashley under went 70 days of chemotherapy called RAPID COJEC a mixture of Vincristine, Carboplatin, Etoposide, Cisplatin and Cyclophosphamide. The neuroblastoma did respond, halving the size of the mass and clearing 95% of the bone marrow, but unfortunately there were still 4 small hot spots that were visible on scans. Ashley then went on the have 4 rounds of a harsher mix of chemotherapy called TVD (Topotecane, Vincristine and Doxorubicin).   • The Mechanism of Cisplatin (New -HD) Cisplatin is in a class of drugs known as platinum-containing compounds used to treat various types of cancers including metastatic testicular and ovarian tumors. The molecule was first discovered in 1845, but did not receive FDA approval until 1978. Today it is known as the "penicillin of cancer drugs," because it is so effective for many different cancers. There are three key players involved in Cisplatin's mechanism: (1) Cisplatin, (2) DNA (3) and an HMG Protein. Most Cisplatin enters the body through active transport, but some molecules are passively defused through the cell membrane. Once in the nucleus, Cisplatin can form an adduct with two consecutive guanine bases within a strand of DNA. The molecule loses its chlorine atoms in exchange for the nitrogen atoms of the target guanines. Cisplatin can bond more tightly with nitrogen because nitrogen balances the platinum charge more effectively than chlorine. It is this adduct-induced DNA bend that allows binding of proteins which contain the high mobility group, HMG domain. Once the protein is bound to the DNA, it inserts a wedge-like phenyl group of phenylalanine 37 into the widened minor groove created by the bend. The tightly bound HMG protein causes destacking of the nucleotide bases, resulting in the DNA helix becoming kinked. Here is some information about breast cancer: • It is the most common form of cancer in women • Some types of breast cancer are sensitive to, and are thus be fed by, female hormones • Treatments include surgery, chemotherapy, radiation, and drugs that block hormones Researchers from the Washington Hospital Center randomly assigned over 5000 women with early-stage breast cancer that was hormone sensitive to one of three chemotherapy regimens involving three drugs that are known together as ACT. The first group received the two drugs doxorubicin and cyclophosphamide followed by docetaxel. The second received doxorubicin and docetaxel together, and the third received doxorubicin, cyclophosphamide, and docetaxel together. After a follow-up of more than six years, a greater proportion of the women who received the sequential therapy were alive, as compared with those who received the doxorubicin and docetaxel regimen. After eight years, a greater proportion of women who received sequential therapy were both alive and cancer-free, as compared with the women in either of the other two treatment groups.
