Severity of sleep apnoea predicts glucose control in type 2 diabetes A recent American study has shown that controlling sleep apnoea in people with type 2 diabetes can be as effective as taking medication. Obstructive Sleep Apnoea (OSA), is a serious condition that can lead to repetitive pauses in breathing during sleep. The condition has been shown to worsen glucose control in people without diabetes and is very common in people with type 2 diabetes. Researchers from the University of Chicago studied 60 people with type 2 diabetes to see if the severity of sleep apnoea was linked to blood glucose control. After controlling for other factors, the researchers found a strong relationship between the severity of sleep apnoea and HbA1c levels. They suggest that the evaluation and treatment of sleep apnoea should become an important factor in the treatment of type 2 diabetes and could even reduce the need for diabetes medication for some individuals.
Obstructive Sleep Apnoea 40% of the local adult population, age between 30 to 60 suffer from a medical condition called Obstructive Sleep Apnoea (OSA). Apnoea is Latin for cessation of breathing and hence OSA is a condition where there is repeated upper airway closure during sleep. A person with OSA is usually unaware of how many times choking and awakenings occur during sleep. OSA sufferers develop a higher risk of contracting chronic diseases such as high blood pressure, heart disease and strokes and diabetes. Even so, 80% of OSA suffers did not seek medical treatment due to low awareness. One of the main symptoms that OSA patients encounter is cessation of breaths for more than 10 seconds. Other symptoms that are often faced include daytime sleepiness, poor concentrate in the day and frequent headaches. Apart from surgery, OSA patients can also use Continuous Positive airway pressure (CPAP) machines to aid better sleeping. CPAP machine works by forcing air into the airway, making sure that breathing is constant. The machine also monitors breathing patterns and adjustments can be made to the breathing. Dr Mark mentioned that OSA is common in the Asian population due to a flatter face structure resulting in a narrower airway. Patients who smoke, drink or are obese have increased risk of OSA. In severe cases of OSA, cessation of breaths can clock up to 600 times, causing increased heart rate and blood pressure. Studies have shown that OSA is present in 70% of stroke patients. Radiofrequency Procedure for Snoring and Mild Obstructive Sleep Apnoea (OSA) Surgical treatment of habitual snoring and mild obstructive sleep apnoea (OSA) can be carried out as an outpatient procedure under local anaesthesia. Options of modalities than can be utilized includes diathermy, sclerosant injection, pillar insertion, radiofrequency,and carbon dioxide (CO2)-laser. The aims are to remove the extra/redundant tissue or to induce stiffening effect which eventually lessens the fluttering and collapse of the palate and uvula onto the posterior pharyngeal wall. These surgical procedure may involve the soft palate, uvula or the palatopharyngeal folds either singly or in combinations depending on the findings. This video shows the outpatient surgical procedure as a treatment of habitual snoring by using a bipolar dual probe radiofrequency device. The tongue was pressed down by using a tongue depressor which was handled by the patient himself. Initial throat assessment was done and followed by local infiltrations of marcain:adrenaline mixture at multiple spots involving the soft palate and palatopharyngeal webs bilaterally. Once the numbing effect gained, the dual probe was gently introduced and pierced the epithelium and finally placed nicely into the mucosa until its insulated part (black colured) reached. The device was set at 10W Auto Stop mode and the foot paddle pressed when ready.
