RE: Hip Replacements and Massage From: Hi, Massage Nerd - I am a massage student close to graduation. Last week in our student clinic we had a woman with 2 hip replacements. She had spent the previous weekend on a 20 hour road trip and felt tight in her hip as a result. Normally, joint play/mobilization would be indicated, but we weren't sure how the hip replacement might affect this technique! We ended up playing it safe and among other techniques performed an iliopsoas release for her, which seemed too help a little. So... mobs for artificial joints... Is it a caution? A contraindication? Would it be beneficial? I don't even know how a replacement femoral head is attached into the acetabulum... is it the same capsule & ligaments or is it all new? Do you have any experience in joint mobilization of artificial joints? Lots of questions to get some discussion going. :) Thanks! Erin

"Anatomy and Physiology", The Joints of the Body, The Hip The Hip is a very complex joint. It incorporates the pelvic gurtle, its' articulation is bound to the pelvis with a complex web of ligaments.The Hip is responsible for the free movement of each of the legs, in their full range of motion. The Femur bone, the large bone of the upper leg has a narrowed area just proximal to the acetabulum, or the "ball" of the hip joint. This area is especially vulnerable to fracture in general, and even more so, in people with osteoporosis. The muscles of the upper leg play a very important role in the control of the legs, and by maintaining strength in the leg muscles, helps to guard the hip joint from excessive stress, and damage.

Original biological reconstruction of the hip in a 4-year-old girl We resected the proximal femur in a 4-year-old child who had Ewing's sarcoma. We reconstructed her hip by an autotransplant of the ipsilateral proximal fibula with its vascular supply, inserted inside a massive bone allograft, and fixed to the residual femur by a plate. Serial postoperative imaging showed the progressive hypertrophy of the transplanted fibula, whose head displayed impressive tridimensional growth inside the acetabulum. 4 years after surgery, hip motility is 100º in flexion, 10º in extension, and 30º in abduction, and the patient's limb is weight bearing.
